Dr. Ben Soffer, PLLC operates as a concierge medicine practice on a membership basis. Membership is subject to availability and acceptance by the Practice. Key membership terms include: - Membership requires a signed Membership Agreement, which contains the complete terms and conditions of membership - Membership fees are separate from and do not include costs for laboratory tests, imaging, specialist referrals, hospitalizations, prescriptions, or other services not directly provided by the Practice - The Practice maintains a limited patient panel to ensure quality care for all members - This is a direct-pay practice; we do not bill insurance for membership fees or routine services - Membership includes a minimum commitment period as specified in your Membership Agreement Complete membership terms, including services included, fees, and cancellation policies, are provided in the Membership Agreement.
